Declaration of participation for parent / legal guardian

 

I, ….., as the parent / legal guardian for the minor declare he / she is in the right physical and mental health to appropriately take the ski or snowboarding lessons organized by Outdoor Stuff NVA SRL.

I declare I have read and acknowledged the “Terms and conditions” published on www.outdoorfun.ro/terms-and-conditions

I declare I have acknowledged and I and the minor will obey the following:

• I fully understand the degree of risk and physical fitness required to participate into the ski and snowboarding lessons;
• The minor has the appropriate gear (ski- or snowboarding- helmet, googles, gloves, jacket and pants); I ensured the minor is appropriately dressed for the weather conditions;
• The minor will respect and follow all the safety instructions provided by the ski or snowboarding instructor during the entire lesson;
• I will disclose any medical conditions or any special diet requirements for the minor to the ski or snowboarding instructor before the start of the lesson;
• It is strictly forbidden attending the ski or snowboarding lessons under the influence of alcohol or illicit drugs;
• I am fully aware of the potential risk of accidents (minors or others) which may occur while practicing skiing or snowboarding and / or during the lessons and I am waving any liability against Outdoor Stuff NVA SRL and / or the ski / snowboarding instructor assigned to the minor.

I do agree the ski or snowboarding instructor shall be filming or taking pictures of the minor during the lessons. These shall be used to review ones` progress and provide feedback on improvement areas. These might also be used to promote the ski and snowboarding lessons on the company website and / or company’s or instructor`s social media accounts.
